Do you have Type 2 Diabetes?

If you have Type 2 diabetes and want to learn how to manage your condition better, you can ask your GP or Practice Nurse to refer you onto a FREE 6-week education programme to learn more about the condition and how to manage it better yourself. 

The course, called DESMOND, delivers education and lifestyle advice and you will attend alongside other people who are in the same position as you.  Your doctor or practice nurse will refer you to this programme.


If you have pre-diabetes, the Healthier You Programme can help you to make changes to your lifestyle.  Your practice nurse or GP will provide you with more information if you fall into this category.

 

 

The programme, delivered by Igneus includes education on healthy eating and lifestyle changes, as well as personalised physical exercise sessions, all of which have been proven to reduce the risk of developing the disease.
